The American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) will sponsor the International Heart & Lung Games at the Johnny Mize Athletic Center campus of Piedmont College in Demorest, Georgia. The International Heart & Lung Games will be held on May 23rd, 24th, and 25, 2003.

This event is for the participation of cardiac and pulmonary rehabilitation programs, their staff and their participants throughout the United States and International countries. Recreational activities will include such events as: golf putting, volleyball, table tennis, horseshoes, shuffleboard, basketball shooting, prediction walk, prediction bike, prediction jog, prediction Nu Step, prediction rower, Game of Knowledge, and golf.

RELEASE, COVENANT NOT TO SUE, AUTHORIZATION AND WAIVER

The undersigned hereby agrees that for the consideration of the AACVPR, allowing the undersigned to participate in the International Heart & Lung Games, for which or in connection with which the AACVPR and Piedmont College has made available any equipment, facilities, grounds, volunteers or personnel, the undersigned does hereby release and forever discharge the AACVPR and Piedmont College, its affiliates, trustees, officers, agents, volunteers and employees, of and from any and all claims, demands, rights and causes of action of whatever kind of nature including, but not limited to, negligence, arising from or by reason of any and all known an unknown, foreseen and unforeseen bodily and personal injuries, damage to property and the consequences thereof resulting from participating at or in any way connected to the International Heart & Lung Games. I therefore release, waive and discharge the AACVPR and Piedmont College, and agree to hold harmless from any and all claims and demands whatsoever for compensation or for any other remuneration, as well as any and all claims or demands arising from the International Heart & Lung Games.

I hereby give my consent to and release for the taking, processing and use of any photographs and/ or video tapes of me during my attendance at the International Heart & Lung Games by persons or parties authorized by AACVPR and Piedmont College, for the public or private media, professional or lay publications, art, advertising, trade purposes or for any other lawful purpose whatsoever. I hereby consent to and give AACVPR and Piedmont College the right and permission to copyright, reproduce, publish or use all such photographs and/ or video tapes, or any reproduction thereof, and waive any right that I may have to inspect or approve any such photographs and/ or videotapes or the use of finished product to which such photographs and/ or videotapes may be applied. I also hereby release, waive and discharge the AACVPR and Piedmont College and agree to hold harmless from any and all claims and demands whatsoever for compensation or for processing, use or utilization of any and all such photographs and/ or videotapes. I hereby consent to and agree that any and all such photographs and/ or videotapes become the property of AACVPR and Piedmont College or such persons or parties authorized by these organizations. I also hereby express that I have the right and authority to enter into, execute and grant this authorization and release. By signing this document, I hereby acknowledge that I have carefully read it before signing, and that I agree to and comply with all of the above this ________day of ___________, 2003.

MEDICAL ACKNOWLEDGEMENT TO PARTICIPATE IN THE INTERNATIONAL HEART & LUNG GAMES

Physicians, registered nurses, and an EMS unit medically supervise the International Heart & Lung Games. It is in the best interest of the participant that he or she is able to safely exercise independently and within his or her prescribed heart rate range. It is important that the participant be able to recognize his or her symptoms or intolerance to exercise.

The above person is requesting to participate in the International Heart & Lung Games. By signing this medical acknowledgement, it is understood that in your medical judgment, this person has no restrictive medical reason that would subject him or her to any undue risk and that the person can safely participate in these recreational activities.
